About Us
Four Federally Qualified Health Centers (FQHCs) have come together to participate in the Medicare Shared Savings Program under the umbrella of CCN Accountable Care Organization (ACO). The goal of the ACO is to provide our Medicare beneficiaries with coordinated and optimized care, improving their health outcomes while reducing unnecessary costs. Through this partnership, the ACO leverages the expertise and resources of these community health centers to deliver high-quality, patient centered care that ensures our beneficiaries receive the right care at the right time.
